CGM announcement

The PMH Diabetes Clinic welcomes the recent Federal Government announcement making fully subsidised continuous glucose monitoring (CGM) available to young people with Type 1 Diabetes.

We strongly support this decision and look forward to working with eligible families. We understand that many of you will be keen to take advantage of this benefit as soon as possible. 

The Diabetes Team is dedicated to assisting you but please be mindful that it has limited resources and needs to continue with our regular activity such as looking after newly diagnosed and unwell patients, attending to clinic, and insulin pump starts and upgrades.

To assist all parties, we suggest that you familiarise yourself with the eligibility criteria via the link below.  Please note that this link includes access to an application form you will need to complete.

***YOU MUST be registered with the NDSS to access the CGM program***

It is important that you make an informed decision about which device you and your child would like to use prior to completing your application form. Each CGM device has different features and capabilities and it is important that the device chosen will meet your goals and expectations. Please find below links to the devices currently available through the scheme. Alternatively, feel free to contact the company representatives to find out more about the device or speak to your diabetes educator.

The application process will depend on whether or not your child fits Category A or B criteria and/or if they plan on continuing to use a CGM device that they have used previously.

 My child is 11 years old and over

We have used CGM previously, and plan to   continue using the same CGM device through the new scheme

  • Complete NDSS CGM Eligibility Form.
  • Email the form to: PMH.DiabetesCGM@health.wa.gov.au and specify which device you would like to use. A member of your clinic team will sign off according to Category A Criteria and the form will be emailed back to you.
  • You will need to send the completed form to   NDSS with your nominated NDSS pick up point.
  • The CGM device will be delivered directly to   the nominated pharmacy.
  • No follow up CGM appointment required through PMH.

 We have used CGM previously, and plan to continue using the same CGM device through the new scheme

  • Complete NDSS CGM Eligibility Form.
  • Bring the form to your next diabetes clinic. The clinic team will determine your eligibility according to Category B Criteria and will sign off if appropriate.
  • Once the form is completed, you will need to send it to NDSS with your nominated NDSS pick up point.
  • The CGM device will be delivered directly to   the nominated pharmacy.
  • No follow up CGM appointment required through PMH.

 We have not used CGM before OR we plan on using a CGM device which we have never used before

***Please DO NOT call the emergency paging system to access this scheme. The doctors carrying the emergency page are not able to assist accessing the CGM scheme and extra calls about non-urgent matters mean there will be additional delays returning calls of an emergency nature to families.***
