People with type 1 diabetes require the completion of a Medical Assessment form for activities such as driving, skipper tickets and scuba-diving etc. This ensures that the patient, and members of the public, are kept safe whilst participating in particular activities such a driving a vehicle or a boat.
This form requires a proper medical assessment by one of our clinic staff and should be completed during a scheduled visit, unfortunately it is not as simple as a doctor signing a form.
We cannot complete assessments over the phone, or via our email triage service, as the doctor will not be able to review the patient. It is advised for patients to bring these forms to their next clinic visit.
